Client Forms
Effective March 6, 2023, paperwork must be submitted for every reservation. Please complete all your paperwork online before check-in.
If you prefer to download and print your paperwork, you can do that directly from our website. If so, feel free to scan your paperwork and email it to us at raezorgbd.office@gmail.com, or you can bring it with you at check-in. We also have printed copies of the paperwork available at the front desk. If we do not receive all the paperwork ahead of time, we will prepare any missing paperwork for you to fill out at check-in. All the paperwork could take up to 20 minutes to complete, so please plan accordingly.
Owners are responsible for providing us a copy of your most current vaccination records. All vaccines must be up to date with our requirements no later than seven (7) days prior to check-in for all boarding reservations. You can attach your vaccination records to your paperwork or email them to us at raezorgbd.office@gmail.com.

- All Animals Veterinary Hospital Drop-Off Form — If your pet will be going to All Animals Veterinary Hospital while they are in our care, this form must be filled out before your pet's appointment.
- Client Registration — This form must be completed each year.
- Dog Boarding Contract — This form must be completed for each boarding reservation.
- Medication/Supplement Form — This form must be completed for each boarding reservation during which your pets will be taking medications, supplements, and/or vitamins. As a reminder, Dasuquin, Cosequin, FortiFlora, Fish Oil, and Calming Chews will all be billed as medications. If your pets will not be taking medications, supplements, or vitamins during their boarding stay, you do not have to submit this form.
