Updated Vaccine & Paperwork Policy
Effective November 6, 2024
We will not contact veterinary offices to retrieve vaccination records. Due to new privacy restrictions, owners are now fully responsible for providing us with your most current vaccination records. You can attach your vaccination records to your online paperwork, email them to us at raezorgbd.office@gmail.com, or drop them off at the front desk. Many local vets have our email on file, so you can also ask them to send us your vaccination records.
We will not make boarding reservations until all paperwork is completed and owners have provided records showing their pets are up-to-date with our vaccination/examination requirements. All vaccinations/examinations must be up-to-date with our requirements no later than seven (7) days before check-in. Paperwork can be completed online; we also have printed copies available at the front desk. Paperwork must be completed before your check-in day. All holiday/summer boarding reservations require a non-refundable deposit that must be paid before we will make the reservation. Refer to our pricing sheets for our deposit schedule.
- Client Registration — This form must be completed each year.
- Dog Boarding Contract — This form must be completed for each boarding reservation.
- Medication/Supplement Form — This form must be completed for each boarding reservation during which your pets will be taking medications, supplements, and/or vitamins. As a reminder, Dasuquin, Cosequin, FortiFlora, Fish Oil, and Calming Chews will all be billed as medications. If your pets will not be taking medications, supplements, or vitamins during their boarding stay, you do not have to submit this form.
Effective August 5, 2024, we do not accept unneutered males for boarding. All males must be neutered to board with us. Existing boarding clients before August 5, 2024, and daycare dogs under one year of age are exempt from this requirement.

Effective August 5, 2024, we do not accept unneutered males after one year of age for daycare. All adult males must be neutered for daycare. Existing daycare clients before August 5, 2024, are exempt from this requirement.

- An up-to-date Rabies vaccination is required for grooming. We highly recommend an up-to-date Bordetella vaccination.
