Client Forms

  1. Client RegistrationThis form must be completed each year.
  2. Dog Boarding ContractThis form must be completed for each boarding reservation.
  3. Medication/Supplement Form — This form must be completed for each boarding reservation during which you will be bringing medications, supplements, and/or vitamins for your pets, even if the medications/supplements are packed in your pet's food. As a reminder, Dasuquin, Cosequin, FortiFlora, Fish Oil, and Calming Chews will all be billed as medications.

All medications/supplements must be kept in their original containers, with labels that align with the veterinarian's prescribed dosage. Instructions on the medication/supplement sheet must also match the veterinarian's prescribed dosage and the instructions on the label. If multiple pets are sharing a medication, the label must include all pets' names and their respective dosages. Please do not pre-pack your pet's medications or supplements with their food or put them in pill containers.

Our facility is required to comply with regulations set forth by the North Carolina Department of Agriculture and Consumer Services under the ANIMAL WELFARE SECTION.

(j)  Boarding kennels shall not administer a prescription medication, tranquilizer, sedative, or any pharmaceutical drug designed to calm an animal unless the medication or drug is administered under the direction of or by prescription from the animal's veterinarian, and written permission from the animal's owner. In the event a boarding kennel agrees to administer such medications or substances, the medications shall be in the original container issued by the veterinarian or pharmacy and administered according to label directions. The administration of these medications or substances shall be documented as required by 02 NCAC 52J .0102.



Effective August 5, 2024, we do not accept unneutered males for boarding. All males must be neutered to board with us. However, existing boarding clients who have regularly boarded with us and maintain an active account will be exempt from this requirement. Please note that accounts that have not used our services in over a year will be considered inactive, and any unneutered male pets will not be allowed to board. Additionally, daycare dogs under one year of age are exempt from this requirement.

Effective August 5, 2024, we do not accept unneutered males after one year of age for daycare. All adult males must be neutered for daycare. However, existing daycare clients who have regularly used our daycare services prior to this policy change and have maintained an active account will be exempt from this requirement. Please note that accounts inactive for over a year will not qualify for this exemption, and any unneutered male dogs will not be allowed in daycare. Additionally, male dogs under one year of age are exempt from this policy.

  • An up-to-date Rabies vaccination is required for grooming. We highly recommend an up-to-date Bordetella vaccination.
